One of the recommendations of this Global Patient Safety Challenge was the adoption of a checklist for use in surgical procedures. Pointing to the ubiquity of the use of surgery in both the developed and developing world, WHO Patient Safety aimed to meet four main problems: a lack of awareness of the issue a lack of data on surgical complications inconsistent use of available safety resources and increasing complexity of surgical procedures. In 2004, an estimated 187 to 281 million surgeries were performed worldwide, with complications occurring in 3-22% and deaths in 0.4-0.8% of procedures the death rate in major procedures rose to 5-10% in developing nations. In January 2007, an international consultation meeting was held on the second Global Patient Safety Challenge, called Safe Surgery Saves Lives. In 2005, WHO Patient Safety began issuing Global Patient Safety Challenges, which bring together teams of specialists in order to put together clinical guidelines and tools for research that address patient safety issues, such as hand washing. The Global Initiative for Emergency and Essential Surgical Care and the Guidelines for Essential Trauma Care focused on access and quality. In 2004, the World Health Assembly (WHA) founded the WHO Patient Safety international alliance in order to tackle issues of adverse effects in unsafe healthcare. While the checklist has been widely adopted due to its efficacy in many studies as well as for its simplicity, some hospitals still struggle with implementation due to local customs and to a lack of buy-in from surgical staff. Several studies have shown the checklist to reduce the rate of deaths and surgical complications by as much as one-third in centres where it is used. It is one affordable and sustainable tool for reducing deaths from surgery in low and middle income countries. The checklist serves to remind the surgical team of important items to be performed before and after the surgical procedure in order to reduce adverse events such as surgical site infections or retained instruments. The World Health Organization (WHO) published the WHO Surgical Safety Checklist in 2008 in order to increase the safety of patients undergoing surgery. ![]() BWS values decreased during FLU treatment temporal period. Lormetazepam concentrations were 502.5 ± 610.0 ng/ml at hospital admission, 26.2 ± 26.8 ng/ml after 4 days, and 0 at the end of treatment. Serum FLU concentrations significantly decreased from 0.54 ± 0.33 ng/ml (mean ± SD) after 4 days of treatment to 0.1 ± 0.2 ng/ml at the end of infusion. Results: Fourteen patients (mean age ± SD 42.5 ± 8.0 years, 5 male and 9 female), admitted to the hospital for high-dose BZD detoxification, were enrolled in the study. Changes in withdrawal severity were assessed by using the BZD Withdrawal Scale (BWS). Methods: Seven-day FLU 1 mg/day subcutaneously injected through an elastomeric pump and BZDs lormetazepam, clonazepam, and lorazepam were assessed by HPLC-MS/MS in serum of patients before and after 4 and 7 days of FLU continuous infusion treatment. The aim of this study was to confirm FLU efficacy for reducing BZD withdrawal syndrome by means of continuous elastomeric infusion, correlated to drugs plasma level and patients' compliance. Studies in chronic users who discontinued BZD in a clinical setting suggested that multiple slow bolus infusions of FLU reduce BZD withdrawal symptoms. 3Department of Internal Medicine, Addiction Unit, Verona University Hospital, Verona, ItalyĪn effective approach in the treatment of benzodiazepine (BZD) overdosing and detoxification is flumazenil (FLU).2Forensic Toxicology Laboratory, Department of Diagnostics and Public Health, University of Verona, Verona, Italy.1Department of Diagnostics and Public Health, University of Verona, Verona, Italy. ![]() Anna Benini 1, Rossella Gottardo 2, Cristiano Chiamulera 1, Anna Bertoldi 3, Lorenzo Zamboni 3 * and Fabio Lugoboni 3 ![]() We will email you (the customer) a return shipping ticket and provide you with a phone number for our carrier to arrange collection.email us at: to discuss return of goods. If you want to return goods because you have changed your mind: At our discretion we will exchange goods or provide credit vouchers to the value of the original purchase providing the goods are returned within 30 days and in the original state they were sold to you. Schoenen and his colleagues have long argued that migraineurs have a deficit in habituation that is revealed in the cortical pattern-reversal visual evoked potential (PR-VEP) during extended viewing of a high contrast phase-reversing checkerboard stimulus ( 12– 14) but see ( 15)). Moreover, an extensive electrophysiological literature has clearly indicated that it is repetitive stimulation, not a brief stimulus, that reveals the truly striking differences between migraineurs and individuals with no migraine history. However, it has long been clear that the problem for migraineurs comes principally from higher contrast stimuli, not from stimuli that are barely detectable. Most psychophysical work on flicker or temporal frequency has focused on detection thresholds – the minimum contrast that can be detected against a homogeneous field - and there are mixed results as to whether migraineurs differ from controls ( 9– 11). ![]() In a laboratory test of flicker aversion thresholds, we have repeatedly found that migraineurs choose to abort a flickering screen at lower contrasts than healthy controls who, in many cases, are not bothered at all by such a stimulus even at maximum contrast. While many find the flickering sunlight to be mildly irritating, migraineurs find this much more aversive ( 6). A good illustration of this is driving through an avenue of trees at sunset when the sun falls directly behind the tree line. Flicker is particularly bothersome to individuals with migraine. In particular, spatial and temporal frequencies close to the peak of the human contrast sensitivity function (3–5 c/deg stripes and 10–20 Hz flicker) are most aversive ( 2– 5). However, interictal visual discomfort has been reported to be linked not only to light intensity, but also to specific spatial and temporal properties of the visual input. ![]() While most extreme during migraine episodes, photophobia may also afflict migraineurs between episodes. Photophobia (light avoidance) is characterized by hypersensitivity or intolerance to light, such that moderate intensities are perceived as uncomfortable and even painful ( 1). ![]() The defining visual features of migraine are photophobia and, in a subpopulation of migraineurs, visual aura which typically precedes headache onset. |
